Our Social Workers are here to make a difference!

Horizon Family Medical Group’s Behavioral Health Program has a team of Social Workers embedded within our Primary Care Offices. The Social Workers are part of your medical team and provide short term behavioral health services that address patient’s and family’s overall health and wellbeing. When you think of overall wellness, you may not think of mental and emotional health. However, mental health is just as important as physical health.

Common Symptoms of Behavioral Health Issues:

  • Thoughts of hopelessness or helplessness
  • Reduced Ability to Concentrate
  • Excessive Worrying
  • Irrational Fears
  • Extreme Mood Swings
  • Withdraw from Normal Activities

Studies show that 44 million Americans are suffering from mental illness, but less than half of them are actively receiving care. While almost 20 million Americans are currently suffering from substance abuse disorders and less than 15% of them are receiving treatment. We are here to help you treat and manage behavioral health disorders. Our Licensed Social Workers offer Solution Focused Brief Therapy, a short term therapeutic approach. Please speak with your provider to obtain a referral for clinical behavioral health or case management services. Our case management services can assist with housing, food security, geriatric services, transportation, financial assistance, and long-term counseling.
