Patient Family Advisory Council (PFAC)

Horizon Family Medical Group would like to invite you to be part of our Patient Family Advisory Council (PFAC).

Our PFAC meets quarterly on the Zoom platform to discuss Horizon Family Medical Group as a whole and the ways that we can improve overall patient care and experiences.
